During your initial appointment, your physical therapist will complete a comprehensive evaluation of your condition and identify the source of your discomfort or dysfunction. You will be educated about your condition and, based on your needs, you will receive an individualized plan of care aimed at helping you meet your goals.
Also during your initial appointment, you will begin to use the complementary treatment techniques outlined in your plan of care to start to address the cause of your discomfort, relieve your pain, and restore your function. Your initial appointment will last approximately 30 minutes (with subsequent appointments typically scheduled for 60 minutes). To ensure that you are able to spend the entire 30 minutes of this appointment with your therapist, please plan to arrive 15 minutes before the start of your first appointment to complete your new patient paperwork. Alternatively, you can access the new patient paperwork online and bring the completed forms with you to your first appointment.
For your first visit, please make sure to:
Bring your physician's referral or prescription.
Bring your insurance card or insurance information.
Wear comfortable and accessible clothes so we can better evaluate you.
Complete the necessary new patient paperwork.
Bring MRI results, x-rays, or any other reports that might help your physical therapist learn more about your condition.
If you are unable to keep an appointment, please give us 24 hours' notice so that we may offer your appointment time to another patient.
